Annual Conference Fees 2008

Advance registration is required. Due by the end of August.
Conference fees are non-refundable and must be paid by the Conference date to avoid additional expense fees.

Registration Fee is $xxx before September.
Registration includes the entire conference, shared lodging, and meals, including dinner Friday through lunch Sunday.

Private Lodging with Early Registration is xxx before September.

Spouse/Guest Fee is $xxx per person before September.
A guest doesn’t attend the conference. Guests must share lodging with registered conference participant, meals are included.

Off-Grounds Registration Fee $xxx before September
Includes conference and meals (no lodging).

Provide the name and a contact phone number for the financial person who will be
handling the payment method you choose to use!
This is NOT a recharge event for UCSF~your method of payment requires authorization
directly from your fund manager.


UCB Mentor Faculty: Please, provide students with the Account Chart String information necessary for registration.
Students: Make sure you know the Account Number to be charged.


UCSF Mentor/Faculty will need to instruct their financial assistant for each fund used, to transfer Registration Fee as expense
from the Fund you designate for each person attending.
The financial person should attach copy of your registration confirmation email to the Online (Peoplesoft Journal) transaction.
Payments go to the UCSF Bioengineering DPA 404910 FUND 69712
The Bioengineering Admin Cannot do this for you because of the Financial controls in electronic authorization.


LATE FEES:
Registration made as of the 1st of September increase the expense by $100.
Rates as of September and October Registrations will be:
Full Registration will be $xxx,
Private Lodging will be $xxx
Off-grounds and Guest fees will increase to $xxx per person

• Attendees with available fund sources (i.e. grants,educational allowance) may request conference travel reimbursement through their department.
• Faculty mentors are responsible for the travel and registration fees of the graduate bioengineering students in their labs (as stated in the Group’s mentor financial support requirements).
• Student Fee Waiver is available to eligible incoming first and second year graduate bioengineering students who have no extramural funding, faculty mentor support or are not committed to a lab.

If you have guests or are a Mentor paying for students, please, be prepared to list them.
Students: Please, enter the method of Payment information provided by your mentor.

TRANSPORTATION

These selections are to be made on the Registration Site. Please, make sure you know the following
before visiting the registration site.

Do you have transportation? if Yes, then can you offer Carpool?
If so, then how many, seats are available, and from which campus will you be traveling?

if No, then is a ride needed and if so how many seats?

OR

Are you eligible to volunteer to drive a Rental Van (12 passenger)? IF so from which campus will you be able to travel?
